Bone Marrow Aspirate Concentrate (BMAC)

Bone Marrow Aspirate Concentrate (BMAC) is an advanced orthobiologic treatment that uses a patient’s own bone marrow to help manage pain and support healing. Bone marrow contains a rich mixture of white blood cells (including progenitor cells, lymphocytes, and granulocytes), platelets, growth factors, and proteins. When concentrated, these biologic components may help reduce inflammation, support tissue repair, and restore function in joints, tendons, and ligaments.

 

At Cape Regenerative Medicine, BMAC is offered as a non-surgical option for patients seeking alternatives to steroids, viscosupplementation, or surgery. It is performed as an outpatient procedure under image guidance for precision and safety.

Who may benefit from BMAC?

BMAC may be an option for patients who:

  • Have mild to moderate osteoarthritis of the knee, hip, shoulder, or ankle.

  • Struggle with chronic tendon injuries that have not improved with standard care.

  • Experience joint pain that limits activity despite trying physical therapy, medications, or prior injections.

  • Are not candidates for surgery or prefer to delay surgical treatment.

 

BMAC may be especially appropriate for active individuals who want to maintain mobility and reduce pain while avoiding or postponing joint replacement.

How does the procedure work?

  1. Bone Marrow Collection – Bone marrow is aspirated (usually from the pelvis) using a minimally invasive technique.

  2. Concentration – The aspirate is processed in a specialized system to concentrate beneficial cells, platelets, and growth factors.

  3. Targeted Injection – Under ultrasound guidance, the concentrated bone marrow is precisely injected into the injured joint, tendon, or ligament.

 

The procedure is performed in the office and most patients return home the same day. Downtime is typically minimal, though activity modifications are recommended during recovery.

Contraindications – Who is Not a Candidate?

BMAC may not be appropriate for every patient. It is not recommended for:

  • Patients with an active infection.

  • Patients with a history of cancer within the past 5 years

  • Patients who have not yet attempted standard, conservative therapies such as physical therapy.

  • Other medical conditions or medications may also affect candidacy, which is why a thorough consultation is essential.

 

Conditions MSK Ultrasound Can Diagnose

  • Tendon injuries and tears (rotator cuff, Achilles, biceps, etc.)

  • Ligament sprains and tears

  • Muscle injuries or strains

  • Joint inflammation and arthritis

  • Nerve entrapments (such as carpal tunnel syndrome)

  • Bursitis, cysts, and other soft tissue abnormalities
     

What to Expect After BMAC

 

  • Most patients experience gradual improvement in pain and function over several weeks to months as the body responds to the biologic treatment.

  • The goal of BMAC is to reduce inflammation, improve mobility, and enhance quality of life—not to cure osteoarthritis or completely regrow cartilage.

  • Ongoing lifestyle measures, including physical therapy, weight management, and exercise, remain essential to long-term joint health.
